The Surgery department has an exciting opportunity for a Nurse Navigator 1 (H) position. The incumbent is the initial point of contact for a patient entering the health system and assures timely scheduling of the first appointment, and coordination of care after completion of the first appointment and supports the patient throughout the care continuum. The Nurse Navigator 1 (H) serves as a liaison between patients, families, caregivers, and the multidisciplinary care team. The incumbent demonstrates a commitment to quality patient care, implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices. Core job functions include triaging new patient appointments to the appropriate provider(s) and assuring timely scheduling of initial appointments, reviewing outside medical records for appropriate scheduling, assessing barriers to care and referring to support services, educating on the treatment plan for patients based on diagnosis, supporting patients throughout the care continuum, counseling individuals and patients on positive health practices, collaborating with a multidisciplinary team of experts to outline the best treatment for patients, performing holistic evaluation of specialty population, implementing improvement of patient care and healthcare policies and resources, mentoring other healthcare professionals, maintaining professional knowledge by affiliating with professional and technical organizations, and adhering to University and unit-level policies and procedures. Department specific functions include supporting clinical functions within scope of practice for all Divisions within the Department of Surgery, assisting with all in-person, virtual and/or telephone patient-related clinical questions and concerns, communicating effectively with Physicians and mid-level providers, serving as a clinical resource to surgical scheduling, patient access and other non-clinical teams, building relationships with clinical and non-clinical staff to streamline processes, coordinating care across multiple departments, agencies and/or organizations, completing medical related forms such as FMLA’s and DWC-25 forms, discussing clinical information with insurance companies to obtain necessary approvals for surgery/treatment, contacting outside providers/hospitals to secure medical records, documenting all patient related encounters in electronic medical records, functioning as a professional role model, and supporting other departmental functions as needed.
